Arrowhead and Luxfer continue to seek non-compliant SCBA and paintball cylinders that must be tested to comply with DOT requirements

RIVERSIDE, Calif. (June 1, 2007)—For the past 16 months, Arrowhead Industrial Services, a cylinder test and inspection company, and Luxfer Gas Cylinders have been attempting to locate a limited number of composite SCBA and paintball cylinders that may not have been properly tested.
These cylinders, manufactured by Luxfer, are labeled with hydrostatic test dates ranging from May 2005 through September 2005. Cylinders with test dates that do not fall within this five-month period are not affected.
The majority of the cylinders in question have now been recovered and tested. However, as of June 1, 2007, 1,614 self-contained breathing apparatus (SCBA) cylinders, used primarily for firefighter life support, and 319 paintball cylinders remain to be recovered and tested. There is no charge either for shipping or testing.
The sample labels below show where to look to determine the original hydrostatic test date and serial number of your cylinders.
On May 1, 2006, the U.S. Department of Transportation (DOT) issued a safety notice concerning these cylinders and posted a serial number list and the following statement in the Federal Register: "A person with a listed cylinder should discontinue use of the cylinder and return it to Arrowhead . . . so that autofrettage and hydrostatic test can be completed before its next use." Transport Canada issued a similar statement concerning non-compliant cylinders being used in Canada. If you own any of these affected cylinders, you are legally required to stop using them immediately and have them tested.
The serial numbers of all affected cylinders are available electronically on the Internet at www.luxfercylinders.com/ctest in a special database that will quickly and automatically check your serial numbers to determine which of your cylinders, if any, require testing. Serial numbers of cylinders that have already been retrieved and tested are regularly removed from this database, so there is no need to recheck cylinders that have already been sent in for testing. Also, if you have already used the automated database to determine that your cylinders do not require testing, there is no need to recheck them.
